Client Agreement

This agreement is entered into by Nicol Natale LLC and ______________ [printed client name] on _________ [today’s date].

As an agent of Nicol Natale LLC, Nicol Natale will be performing services including reiki, oracle readings, and/or yoga on me. Nicol is a level II certified reiki practitioner, 200HR registered yoga instructor certified by Yoga Alliance, and oracle card reader. By registering for any online yoga classes with Nicol Natale via Zoom or other online platforms, I acknowledge that I agree to the following terms.

With every offering, Nicol’s intention is to provide emotional, physical, and spiritual support to her clients. Nicol’s belief is that the body has the ability to heal itself and her goal is to help achieve balance of their mind, body, and spirit. Nicol is not a trained healthcare professional or mental health counselor; therefore, I acknowledge that it is my responsibility to consult with my healthcare providers regarding medical advice or mental health support prior to my session with her.

I understand that reiki practitioners, oracle card readers, and yoga instructors do not diagnose conditions, prescribe substances, perform medical treatment, nor replace the treatment of a licensed medical professional. It is suggested that I see a licensed healthcare professional for any physical or psychological ailments I may have, and I understand that reiki, yoga, and oracle cards can compliment any physical or psychological care I may be receiving.

I hereby agree that Nicol Natale LLC is not liable for any negative physical, emotional, or spiritual repercussions following any of my sessions with her. I acknowledge that Nicol Natale is using her best judgement and abilities in any advice or guidance she provides to me, but it is entirely my decision whether to follow through on any of her advice. I acknowledge my overall responsibility to advise Nicol about my levels of comfort throughout my session(s), which may impact my session.

I understand that reiki is a Japanese healing technique used for the purposes of deep relaxation, pain relief, and emotional and energetic support. If I choose to receive reiki remotely or in person, I understand that Nicol will be sending energy to me for the duration of my session(s). I hereby acknowledge that reiki and yoga can involve physical touch, and I will make it known to Nicol prior to our session if I do not wish to be touched during our session. I also understand that I will be fully clothed during our session.

I acknowledge that yoga is a practice that involves mental concentration and physical movement, which may be strenuous and carries risk of injury. I agree to judge my own capabilities and to work according to my own limitations. When practicing yoga online with Nicol Natale, I agree to take full responsibility for my own safety and well-being, for not exceeding my limits, and for any injury or damages that may take place.

I waive and release any claim for injury or damages that might take place on the premise, whether or not sustained during the practice of yoga, against Nicol Natale LLC or any person/entity involved during class. My registration and participation in any online yoga classes with Nicol Natale verifies that I am physically fit to participate in yoga classes and a licensed medical doctor has verified my physical condition for participation in this type of class.

In consideration of the services, information, and support I have received or will hereafter receive from Nicol Natale LLC, I hereby agree to release and forever discharge Nicol Natale LLC, its successors or assigns, and all other persons or entities on its behalf from any and all claims, actions, damages, liability, costs or expenses and attorney fees which are related to, arise out of, or are in any way connected to the services provided by Nicol Natale LLC. 

I agree to hold harmless and indemnify Nicol Natale LLC from any and all liability for damage to any third party resulting from the services to be provided by Nicol Natale LLC. This Client Agreement will be effective and binding upon my heirs, agents, and personal representatives and assigns. I have read this Release prior to signing and I understand its effect. I am aware that by signing this Release I am waiving certain legal rights, which I or my heirs, next of kin, executors, administrators, and assigns may otherwise have had against Nicol Natale LLC.

Having read, completed, and understood the foregoing, I request to receive reiki treatment, oracle card readings, and/or yoga from Nicol Natale LLC. I understand that my practitioner and instructor is providing reiki treatment, oracle card reading, and/or yoga at my request, and is not responsible for the outcome of the reiki oracle card reading, and/or yoga session(s).

All payments must be made at the time of booking. There are no refunds for booked single sessions, but you are able to reschedule if the date that you originally booked has to be changed. You must cancel or reschedule at least 24 hours in advance, or you will be charged for the full session. If you buy a package and do not want to continue to work with Nicol for the remainder of your package, you will be charged for the first session and refunded the rest of your package. If you are buying a single session or package on behalf of someone else, the client who you are gifting the session or package to must complete the Client Agreement form and email it to reikiwithnicol@gmail.com at least 24 hours in advance of their scheduled session.

Any dispute arising under or in relation to this agreement shall be resolved in the courts of the state of Delaware and shall be governed by the laws of Delaware.

Booking a reiki session, oracle card reading, and/or registering for a yoga class with Nicol Natale acts as a binding signature for yourself, your child (under 18), or anyone you are gifting a session to.
